
Valves — Three Axes, Not Thirty Valves: How the Closure Member Moves Decides Everything Else
A line list says "6", Class 300, isolation." Somebody picks a gate valve. Somebody else, two weeks later, needs to control the flow on the same line and picks a second gate valve because that is what is already in the spec. It is installed. It gets left half open.
Six months on, the first valve will not shut tight. Nobody can say when it stopped sealing, because a gate valve that is nearly shut passes a little and looks fine on a gauge.
